For New Patient Registration

Your current postcode must fall within the Practice catchment area.   

How to register as a New Patient

  1. Please bring photographic identity e.g. passport, driving licence. We also need proof of address, for example a utilities bill (gas, electricity, landline telephone) which must be dated from the last three months. 
  2. You will need to fill in a New Patient Registration Card, GMS1 Form, Alcohol Screening Questionnaire Form and Summary Care Records Form. These can be downloaded here or we have copies at reception.
  3. We will not be able to register you unless you are able to fully complete the registration form. We must have the name and address of your last GP including postcode and your last address including postcode. If you have just entered the UK we will need the date of entry. Please also tell us your ethnicity.
  4. Please bring birth certificates and the red book for any children registering.
  5. If you need help completing the form, please do not hesitate to ask the Receptionist. If English is not your first language it may be helpful to ask a friend or relative to help you to complete the registration forms.
  6. A health check will be booked with the nurse or healthcare assistant. All new patients are routinely offered a screening test for HIV.
  7. Unfortunately we will not be able to register you unless you bring all your documents and completed forms.
